We are currently seeking a talented, motivated and detail oriented QDDP to join our highly skilled team. The QDDP  is responsible for leading, directing and monitoring treatment services and actively promoting agency philosophy in the delivery of those services. Handles all case management aspects of assigned clients’ care. Essential job duties and responsibilities are in accordance with Damar’s Strategic Plan and requirements by licensure.

Responsibilities

Quality Services/High Quality Staff
• Completes Independent Living skills
• Represents clients at doctor’s appointments and exchanges information effectively and accurately.
• Maintains service notes, contacts, data sheet creation and data collection.
• Plans and leads Program meetings
• Develops Individualized Program Plans (IPPs) for each assigned client within 7 days admission and update it as needed.
• Collaborates with Residential Managers to ensure execution of unit programming.
• Leads and directs the implementation of IPPs by providing guidance and training to employees for assigned clients.
• Will be writing and updating monthly IPP to ensure they are ready by the 6th of every month.
• Review and sign MHA and update the addendum as needed.
• Will complete an ICAP assessment within 30 days of admission.
• Will complete Ansell Casey, for those who need it (20 days from admit and update annually)
• Monitor the implementation and documentation of the IL goals
• Complete the IL goals.
• Complete IL monthly reports
• Ensure there Family engagement communication / efforts (weekly) per client in a caseload.
• Ensure there is Referral source communication (weekly) per client in a caseload.
• Will request and attend CFTM Quarterly or as needed.
• Represents clients in Court Reviews and IEP/Case Conferences by providing accurate information regarding the client’s treatment progress and areas of need.
• Serves as an active member of an interdisciplinary treatment team by attending and participating in monthly treatment reviews and weekly meetings.
• Participates in psychiatric reviews for each assigned client by providing direct feedback to the psychiatrist during appointments or written feedback through nursing department.
• Completes and follows up on the BDDS application
• Program Training for unit staff.
• Ensures service design and delivery provides each child with active treatment
• Independent living skill training
• Discrepancies or conflict between programming, medical, dietary, and vocational aspects of the child’s assessment and program are resolved.
• Follow-up to recommendations for services, equipment, or programs
• Works hand in hand with IBI specialist and residential Managers-Interdisciplinary collaboration.
• Ensures that Transportation needs are met: i.e. gas cards
• Ensure that Clients are safe to go for an off-ground outing.
• Ensure that Weekend/holiday notes/plan are complete and sent out to the appropriate parties.
• Manages the Allowance program on their units
• Process Incident reports and make contact to the families and referral sources
• Completes CANS assessments for assigned residents in accordance with state regulations
• Completes outing sheets in collaboration with the residential managers.
• Completes and updates mandatory training and other licensure related items in a timely manner.
• Updates appropriate systems (i.e. Welnet etc.) with client information.
• Maintains documentation that is accurate and legible.
• Attends mandatory staff meetings, LTM, and other meetings as assigned.
• Ensures that all activities, services and programs through Damar are conducted in a manner that is sensitive and shows respect for the cultural and ethnic diversity for all Damar clients.
• Completes incident reports, including reporting any suspected exploitation, abuse, or neglect of a client.
• Ensures that the agency meets its quality improvement, evaluation and reporting requirements.
• Actively participates in and/or oversees data collection and the use of data to improve outcomes, practices and service delivery.
• Utilizes data collection in relation to driving and tracking outcomes and achieving agency initiatives and goals.
• Takes pride and ownership in Damar’s facilities and assigned work areas.
• Will adhere to all policies and procedures as defined in the Employee Handbook and Damar Operational Policies and Procedures Manual.
• Performs other duties as assigned

Financial Responsibilities
• Collaborates with Residential Manager to ensure assigned locations remain within budget parameters.
• Seeks the best use of materials, equipment and staff to maximize efficiency and effectiveness.
• Acts as a good steward of Damar and/or client finances.

External Relationships
• Acts as a liaison and consistently communicates with families, professionals, guardians and referring agencies to provide best outcomes for clients. Ensures that these communications are recorded in a timely manner and participates in weekly narrative communication.
• Respects the culture, diversity and rights of all clients, their families and the community.
• Shows initiative by identifying solutions to work towards improving stakeholder satisfaction.

• Remains up to date on treatment best practices to ensure to proactively meet client demands.
• Prioritizes duties in a manner consistent with organizational objectives and growth.

Qualifications

Knowledge/Skills/Abilities
• Ability to follow and execute specific oral and written instructions.
• Ability to communicate effectively, both orally and in writing.
• Ability to promote and maintain positive interaction with all internal and external stakeholders.
• Knowledge of budgets.
• Knowledge and self-awareness of personal bias.
• Demonstrate respect for cultural, ethnic, spiritual and linguistic diversity and values including differences in personal goals, lifestyle choices and varying family backgrounds and histories.
• Ability to obtain and maintain Child and Adolescent Needs and Strengths (CANS) Assessment certification.
• Ability to be on call 24 hours a day, 7 days a week to handle issues as they arise.
• Ability to pass and maintain Therapeutic Crisis Intervention (TCI) certification if working in children’s
residential home/unit licensed by Dept. of Child Services (DCS).
• Ability to physically manage and restrain clients when necessary, including ability to assist in lifting, maneuvering, and physically supporting individuals up to 200 pounds; ability to lift up to 35 pounds.
• Ability to walk, sit, stand, for long periods of time. May also be required to stoop, bend, squat, push, pull, run, kneel and have full dexterity and agility.

Minimum Qualifications
• Must have at least one year of experience in treating DD/ID population or specialized training and meet one of the following:

  • Bachelor’s Degree in education from an accredited program.
  • Bachelor’s Degree or Master’s Degree in social work from an accredited program. 
  • Bachelor’s or Master’s Degree in a human services field (other than social work) and at least three (3) years of experience under the supervision of a qualified social worker OR at least one year experience working directly with persons with ID/DD. 
  • Registered Nurse
  • Therapeutic recreation specialist who is a graduate of an accredited program.
  • Master’s degree in psychology from an accredited program

• Minimum age of 21 per licensure requirements.
• Valid Indiana driver’s license and driving record that meets eligibility requirements of Damar’s insurance carrier (for all positions requiring driving).
• No felony convictions or other criminal offenses involving a dependent population.
• Successful completion of pre-employment drug screen and physical.
