Damar is looking for a talented and motivated Qualified Developmental Disabilities Professional to be responsible for leading, directing and monitoring treatment services and actively promoting agency philosophy in the delivery of those services. Handles all case management aspects of assigned clients’ care while ensuring all functions are in accordance with Damar’s Strategic Plan and requirements by licensure.

Remarkable ways you will be making a difference:

  • Develops Individualized Program Plans (IPPs) for each assigned client within 7 days admission and update it as needed
  • Completes Independent Living skills for the clients
  • Represents clients at doctor’s appointments and exchanges information effectively and accurately
  • Plans and leads Program meetings
  • Leads and directs the implementation of IPPs by providing guidance and training to employees for assigned clients
  • Writes and updates monthly IPP to ensure they are ready by the 6th of every month
  • Ensures there is Family engagement communication / efforts (at a minimum weekly) per client in a caseload
  • Ensures there is Referral source communication (weekly) per client in a caseload
  • Serves as an active member of an interdisciplinary treatment team by attending and participating in monthly treatment reviews and weekly meetings
  • Participates in psychiatric reviews for each assigned client by providing direct feedback to the psychiatrist during appointments or written feedback through nursing department
  • Completes and follows up on the BDDS application and assist with client transitions and discharges
  • Facilitates Program Training for unit staff including Independent living skill training
  • Ensures service design and delivery provides each child with active treatment
  • Along with the Residential Manager and the Lead Staff, promptly resolves any discrepancies or conflict between programming, medical, dietary, and vocational aspects of the child’s assessment and program
  • Completes follow-up recommendations for services, equipment, or programs

What you need to be considered:

  • Must have at least one year of experience in treating DD/ID population or specialized training and meet one of the following:
    • Bachelor’s Degree in education from an accredited program

    • Bachelor’s Degree or Master’s Degree in social work from an accredited program

    • Bachelor’s or Master’s Degree in a human services field (other than social work) and at least three (3) years of experience under the supervision of a qualified social worker OR at least one year experience working directly with persons with DD/ID

    • Registered Nurse

    • Therapeutic recreation specialist who is a graduate of an accredited program

    • Master’s degree in psychology from an accredited program

  • Minimum age of 21 per licensure requirements
  • Valid Indiana driver’s license and driving record that meets eligibility requirements of Damar’s insurance carrier (for all positions requiring driving)
  • Be clear of any felony convictions, substantiated charges of neglect or other disqualifying criminal convictions
  • Successfully complete a pre-employment physical
  • Favorably meet and maintain Damar’s background screening

Essential Job Functions:

Physical: Primary functions require sufficient physical ability and mobility to work in an office setting; to stand or sit for prolonged periods of time; to occasionally stoop, bend, kneel, crouch, reach, and twist; to lift, carry, push, and/or pull light to moderate amounts of weight (up to 35lbs); to operate office equipment requiring repetitive hand movement and fine coordination including use of a computer keyboard; to travel to other locations using various modes of private and commercial transportation; and to verbally communicate to exchange information.

Vision: See in the normal visual range with or without correction.

Hearing: Hear in the normal audio range with or without correction.
