The Case Manager will be responsible for leading, directing and monitoring treatment services and actively promoting agency philosophy in the delivery of those services. Essential job duties and responsibilities are in accordance with Damar’s Strategic Plan and requirements by licensure.

Remarkable way you will be making a difference...

  • Conducts functional assessment and develops Individual Crises Management Plan (ICMPs) for each assigned client upon intake and updated as needed.
  • Leads and directs the implementation of ICMPs by providing guidance and training to employees for assigned clients.
  • Participates in psychiatric reviews for each assigned client by providing direct feedback to psychiatrist during appointments or written feedback through nursing department.
  • Completes CANS assessments for assigned residents in accordance with state regulations.
  • Oversees Family Time scheduling and encourages Family involvement.
  • Organizes program meetings and CFTM when necessary.
  • Attends mandatory staff meetings, program meetings and other meetings as assigned.
  • Adheres to all policies and procedures as defined in the Employee Handbook and Damar Operational Policies and Procedures Manual.
  • Ensures that the agency meets its quality improvement, evaluation and reporting requirements.
  • Actively participates in and/or oversees data collection and the use of data to improve outcomes, practices and service delivery.
  • Completes and participates in all intakes and discharges for assigned clients.
  • Conducts and documents a comprehensive Mental health Assessment (MHA).
  • Responsible for Independent Living and education coordination.
  • Updates appropriate systems (i.e. MedEz, IARRCA website, etc.) with client information.
  • Represents clients at doctor’s appointments and exchanges information effectively and accurately.
  • Represents clients in Court Reviews, Child and Family Team meetings, and IEP/Case Conferences by providing accurate information regarding the client’s treatment progress and areas of need.

What you need to be considered...

  • Bachelors Degree in social work, psychology, counseling, or related area of study from an accredited school.
  • At least one year of experience in treating DD/ID Population or specialized training preferred.
  • Minimum age of 21 per licensure requirements.
  • Valid Indiana driver’s license and driving record that meets eligibility requirements of Damar’s insurance carrier (for all positions requiring driving).
  • Successful completion of pre-employment physical.
  • Must meet background screening requirements of program(s) assigned to.

Essential Job Functions:

Physical: Primary functions require sufficient physical ability and mobility to work in an office setting; to stand or sit for prolonged periods of time; to occasionally stoop, bend, kneel, crouch, reach, and twist; to lift, carry, push, and/or pull light to moderate amounts of weight (up to 35lbs); to operate office equipment requiring repetitive hand movement and fine coordination including use of a computer keyboard; to travel to other locations using various modes of private and commercial transportation; and to verbally communicate to exchange information.

Vision: See in the normal visual range with or without correction.

Hearing: Hear in the normal audio range with or without correction.
