Damar is looking for a talented and motivated Case Manager to join our highly skilled team. This person will be responsible for assisting young people develop the necessary skills via experimental learning in order to enhance self-sufficiency. Services that could be provided to clients include Educational, Vocational and Employment, Health, Housing, Life and Social Skills, and Youth Development.

  • Provides age-appropriate education regarding life skills needed to live independently, this includes but is not limited to tutoring, mentoring, health care, self-esteem building, life interest explorations and personal relationship.
  • Assists and teaches clients about maintaining a safe and healthy environment, including performing a variety of tasks related to housekeeping including cleaning, washing soiled clothing, changing bed linen, folding and sorting clean laundry.
  • Remains awake and engaged at all times – competently supervising and monitoring the clients. Never leaving clients unattended unless permitted for periods of time based on a client’s service agreement.
  • Effectively responds to and manages crisis situations, which may include client physical aggression, utilizing prescribed verbal intervention or physical management techniques.
  • Completes all required documentation on a daily basis, in a professional manner – objective, accurate and thorough - to provide consistent and accurate for client treatment.
  • Attends mandatory staff meeting and other meetings as assigned.
  • Performs other duties as assigned.
  • Operates Damar vehicles and/or personal vehicles to transport clients or perform other work related duties.
  • Actively participates in and/or oversees data collection and the use of data to improve outcomes, practices and service delivery.
  • Shows initiative by identifying solutions to work towards improving stakeholder satisfaction.
  • Consistently implements behavior management plans to reduce client’s maladaptive behaviors and promote appropriate social behaviors.
  • Observes, records, and reports the clients’ condition and behavior to determine if clients are achieving outlined goals and outcomes from the clients’ service agreement.
  • Commits to memory and implements all client goals and staff interventions as written in the client plans of service.
  • Understands and follows the rules governing client service documentation, including but not limited to medication logs, incident reports, progress notes and all other documentation assigned.
  • Understands client’s rights and protects them by promptly reporting any violations.
  • Completes incident reports, including reporting any suspected exploitation, abuse, or neglect of a client.
  • Will adhere to all policies and procedures as defined in the Employee Handbook and Damar Operational Policies and Procedures Manual.
  • Ensures that the agency meets its quality improvement, evaluation and reporting requirements.
  • Prioritizes duties in a manner consistent with organizational objectives and growth.
  • Completes and updates mandatory training and other licensure related items in a timely manner.
  • Maintains documentation that is accurate and legible.
  • Utilizes data collection in relation to driving and tracking outcomes and achieving agency initiatives and goals.
  • Actively participates in and/or oversees data collection and the use of data to improve outcomes, practices and service delivery.
  • Seeks the best use of materials, equipment and staff to maximize efficiency and effectiveness.
  • Ensures that all activities, services and programs through Damar are conducted in a manner that is sensitive to and shows respect for the cultural and ethnic diversity for all Damar clients.
  • Partners with clients, their families, professionals, and the community in a courteous and respectful manner to provide optimal experimental learning experiences.
  • Takes pride and ownership in Damar’s facilities and assigned work areas.
  • Acts as a good steward of Damar and/or client finances.
  • Respects the culture, diversity and rights of all clients, their families and the community.

What you need to be considered...

 

Education: Bachelor’s degree in social work, psychology, sociology, or a directly related human service field.

 

Experience: 3 years of formal experience in this field.

 

Equipment Familiarity: Proficiency with MS Office applications including: Word, Excel, Outlook and PowerPoint.

 

Certifications, Licenses or Special Training:

  • Minimum age of 21.
  • Must meet and maintain background screening requirements for all Damar Programs as detailed in policy 1.HR.02 Pre-Employment and Employee Background Checks.
  • Valid Indiana driver’s license and driving record that meets eligibility requirements of Damar’s insurance carrier (for all positions requiring driving).
  • Must have reliable transportation and maintain personal auto insurance coverage.
  • Successful completion of pre-employment physical required. 

 

Abilities:

  • Ability to follow and execute specific oral and written instructions.
  • Ability to communicate effectively, both orally and in writing.
  • Ability to promote and maintain positive interaction with all internal and external stakeholders.
  • Knowledge of DCS Chafee Foster Care Independence Program (CFCIP) service standard and associated regulations of services being provided.

 

Essential Job Functions:

Physical: Primary functions require sufficient physical ability and mobility to work in an office setting; to stand or sit for prolonged periods of time; to occasionally stoop, bend, kneel, crouch, reach, and twist; to lift, carry, push, and/or pull light to moderate amounts of weight (up to 35lbs); to operate office equipment requiring repetitive hand movement and fine coordination including use of a computer keyboard; to travel to other locations using various modes of private and commercial transportation; and to verbally communicate to exchange information.

Vision: See in the normal visual range with or without correction.

Hearing: Hear in the normal audio range with or without correction.
