Remarkable way you will make a difference...

  • Provides supervision and guidance to the nursing staff and health specialist team as directed by and in collaboration with the Director of Nursing.
  • Oversees nursing and health specialist personnel scheduling.
  • Maintains on-call availability for the Damar residential health services team.
  • Reviews medication usage, medical logs and condition of medication storage.
  • Schedules and assists with physician, clinic and dental visits as necessary.
  • Collaborates with HR to schedule, administer and document TB testing for employees.
  • Communicates changes in client’s status (legal, psychiatric, physical) with attending physician.
  • Attends mandatory staff meetings and other meetings as assigned.
  • Conducts periodic training for staff as needed.
  • Provides client physical assessment, medical care and necessary follow-up.
  • Attends meetings/functions as requested by the Executive Director of Medical Services.
  • Participates in the orientation of new hires.
  • Teaches Core A and Core B classes to residential, CLASS, and group home staff. Serves as a trainer for the Indiana Core A and B curriculum.
  • Ensures that health-related policies and procedures are followed by the Damar Health Services staff, including residential nurses, school nurses, and health specialists.
  • Oversees client health records and health-related communications to ensure documentation complies with HIPPA and other regulatory standards.
  • Collects and analyzes quality data related to client health. This data includes medication variances, infections, and healthcare quality metrics defined by external licensing agencies.
  • Adheres to all policies and procedures as defined in the Employee Handbook and Damar Operational Policies and Procedures Manual.
  • Completes incident reports, including reporting any suspected exploitation, abuse, or neglect of a client.
  • Ensures that the Agency meets its quality improvement, evaluation and reporting requirements.
  • Utilizes data collection in relation to driving and tracking outcomes and achieving agency initiatives and goals.
  • Completes and updates mandatory training and other licensure related items in a timely manner.
  • Maintains documentation that is accurate and legible.
  • Actively participates in and/or oversees data collection and the use of data to improve outcomes, practices and service delivery under the direction of the Executive Director of Health Services.
  • Ensures the nursing department is up to date on necessary training including vaccine training, TB and CPR certification, Core A and B (if required), and other trainings as required.
  • Audits charts and medication records to ensure accurate and timely documentation by health services staff including nurses and health specialists.
  • Ensures all activities, services and programs through Damar are conducted in a manner that is sensitive to and shows respect for the cultural and ethnic diversity for all Damar clients.
  • Serves as a member of the interdisciplinary team.
  • Takes pride and ownership in Damar’s facilities and assigned work areas.
  • Performs other duties as assigned.

What you need to be considered...

  • Minimum age of 21 per licensure requirements.
  • Must have an active Indiana RN nursing license. Experinced LPNs may also be considered.
  • Therapeutic Crisis Intervention (TCI) training is required, certification is preferred but not required.
  • Ability to pass and maintain CPR Certification. 
  • Must meet and maintain background screening requirements for all Damar Programs as detailed in policy 1.HR.02 Pre-Employment and Employee Background Checks.
  • Valid Indiana driver’s license and driving record that meets eligibility requirements of Damar’s insurance carrier (for all positions requiring driving).
  • Successful completion of pre-employment drug screen and physical required. 
  • One year of experience working with nuropsychiatric or behavioral challengesand/or individuals identifying with the DD/ID population preferred.

Essential Job Functions:

Physical: Primary functions require sufficient physical ability and mobility to work in an office setting; to stand or sit for prolonged periods of time; to occasionally stoop, bend, kneel, crouch, reach, and twist; to lift, carry, push, and/or pull light to moderate amounts of weight (up to 35lbs); to operate office equipment requiring repetitive hand movement and fine coordination including use of a computer keyboard; to travel to other locations using various modes of private and commercial transportation; and to verbally communicate to exchange information.

 

Vision: See in the normal visual range with or without correction.

 

Hearing: Hear in the normal audio range with or without correction.
