Damar is looking for a talented and motivated Case Manager to be responsible for leading, directing and monitoring treatment services and actively promoting agency philosophy in the delivery of those services. Handles all case management aspects of assigned clients’ care.

  • Qualified Developmental Disabilities Professional: The QDDP is responsible for leading, directing and monitoring treatment services and actively promoting agency philosophy in the delivery of those services. Handles all case management aspects of assigned clients’ care while ensuring all functions are in accordance with Damar’s Strategic Plan and requirements by licensure.
  • Case Manager:  The Case Manager is responsible for leading, directing and monitoring treatment services and actively promoting agency philosophy in the delivery of those services. Essential job duties and responsibilities are in accordance with Damar’s Strategic Plan and requirements by licensure.

  • Independent Living skills
  • Represents clients at doctor’s appointments and exchanges information effectively and accurately.
  •  Maintains service notes, contacts, data sheet creation and data collection.
  • Plans and leads Program meetings
  • Develops Individualized Program Plans (IPPs) for each assigned client within 7 days admission and update it as needed.
  • Collaborates with Residential Managers to ensure execution of unit programming.
  • Leads and directs the implementation of IPPs by providing guidance and training to employees for assigned clients.
  • Will be writing and updating monthly IPP to ensure they are ready by the 6th of every month.
  • Review and sign MHA and update the addendum as needed.
  • Will complete an ICAP assessment within 30 days of admission.
  • Will complete Ansell Casey, for those who need it (20 days from admit and update annually)
  • Monitor the implementation and documentation of the IL goals
  • Complete the IL goals.
  • Complete IL monthly reports
  • Ensure there Family engagement communication / efforts (weekly) per client in a caseload.
  • Ensure there is Referral source communication (weekly) per client in a caseload.
  • Will request and attend CFTM Quarterly or as needed.
  • Represents clients in Court Reviews and IEP/Case Conferences by providing accurate information regarding the client’s treatment progress and areas of need.

What you need to be considered...

  • Be at least 21
  • Must have at least one year of experience in treating DD/ID population or specialized training and meet one of the following:
    • Bachelor’s Degree in education from an accredited program.

    • Bachelor’s Degree or Master’s Degree in social work from an accredited program. 

    • Bachelor’s or Master’s Degree in a human services field (other than social work) and at least three (3) years of experience under the supervision of a qualified social worker OR at least one year experience working directly with persons with DD/ID. 

    • Registered Nurse

    • Therapeutic recreation specialist who is a graduate of an accredited program.

    • Master’s degree in psychology from an accredited program

  • Depending on the specific role you may need to possess a valid Indiana driver’s license and good driving record
  • Successfully complete a pre-employment physical.
  • Favorably meet and maintain Damar’s background screening
  • Be clear of any felony convictions, substantiated charges of neglect or other disqualifying criminal offenses

Essential Job Functions:

Physical: Primary functions require sufficient physical ability and mobility to work in an office setting; to stand or sit for prolonged periods of time; to occasionally stoop, bend, kneel, crouch, reach, and twist; to lift, carry, push, and/or pull light to moderate amounts of weight (up to 35lbs); to operate office equipment requiring repetitive hand movement and fine coordination including use of a computer keyboard; to travel to other locations using various modes of private and commercial transportation; and to verbally communicate to exchange information.

Vision: See in the normal visual range with or without correction.

Hearing: Hear in the normal audio range with or without correction.
