What would I be doing?

The Nurse is responsible for delivering quality nursing care to clients of Damar, assessing clients, managing appointments, and administering treatments and medications.

  • Provides timely nursing care to clients of Damar.
  • Ensures provider orders are followed to deliver care and results timely and efficiently.
  • Follows through on referrals, education, and coordination of care until client medical needs are met.
  • Maintains accurate records of client appointments and annual requirements.
  • Maintains on-call availability as needed.
  • Reviews appointment schedule each day and prepares for appointments by ensuring all necessary paperwork is completed by the medical team and is ready prior to the appointment.
  • Completes assessments at the time of admission and annually, and documents findings timely.
  • Tracks and administers client immunizations and TB tests upon admission, annually, and as needed.
  • Attends medical appointments with clients both on campus and at external offices as needed.
  • Completes in-office testing and assists with referrals as ordered by the provider.

What you need to be considered...

  • Must have Indiana nursing license as a Licensed Practical Nurse.

  • Minimum of one year of experience in nursing preferred.
  • Experience working with individuals with neuropsychiatric or behavioral challenges and/or individuals identifying with the DD/ID population.
  • Minimum age of 21 per licensure requirements.
  • Must meet and maintain background screening requirements of program(s) assigned to.
  • No felony convictions or other criminal offenses involving a dependent population.
  • Valid Indiana driver’s license and driving record that meets eligibility requirements of Damar’s insurance carrier (for all positions requiring driving).
  • Successful completion of pre-employment physical.

Essential Job Functions:

Physical: Primary functions require sufficient physical ability and mobility to work in an office setting; to stand or sit for prolonged periods of time; to occasionally stoop, bend, kneel, crouch, reach, and twist; to lift, carry, push, and/or pull light to moderate amounts of weight (up to 35lbs); to operate office equipment requiring repetitive hand movement and fine coordination including use of a computer keyboard; to travel to other locations using various modes of private and commercial transportation; and to verbally communicate to exchange information.

Vision: See in the normal visual range with or without correction.

Hearing: Hear in the normal audio range with or without correction.
